What You Will Do

  • Work in a distinctively different model of primary health.
  • Be a key member of a team who will practice a whole-person approach to build and manage the primary health member population.
  • Collaborate across clinical and non-clinical care team members, including a team of community partners.
  • Provide care to members virtually and in-person.
  • Collaboratively manage a panel of members to holistically maintain their health through longitudinal primary health, care management, and care coordination.
  • Provide comprehensive care management, chronic disease management, urgent home-based and community-based primary health visits, preventative care and wellness, liaison with others around behavioral health and long-term service and support needs.
  • Perform episodic urgent medical/behavioral health visits and/or telephone calls for members to avoid emergency department visits or hospitalization. 
  • Help to sustain a culture centered on value-based care delivery, orienting teams towards opportunities to engage members longitudinally and invest in community-based interventions to improve outcomes, in particular reducing avoidable hospitalizations and downstream progression of chronic disease.
  • Serve as the clinical expert in the care of members with complex needs, and drive the integration of palliative care, primary behavioral health care, and substance use disorder screening and treatment as part of normative primary care.

  • An MD or DO Board Certified in Family Medicine with an unrestricted license to practice in Ohio and an active DEA Controlled Substance Registration or eligible for application.
  • An independent self-starter, a leader, and a strategic thinker who is excited about the big picture of whole health, and the ongoing evaluation and iteration of our care model.
  • Able to talk, type, and document in the moment. 
  • Willing to work a flexible schedule that includes one late evening a week and a rotating on-call schedule.
  • Experienced in providing clinical services to individuals with both chronic medical and behavioral health conditions, and has interest in serving complex, vulnerable, and disabled populations.
  • Adaptable, organized, efficient, a problem-solver, a leader, a strategic thinker, and a mentor, who is excited about the big picture of whole community health.
